Successful Total Arterial Revascularization
Surgery [At Lilavati Hospital]

A 50 year old parsi gentlemen, non hypertensive
and non Diabetic, was admitted in Lilavati Hospital with
symptoms of chest pain and heaviness after food.
He had no past history of IHD / Stroke / TIA / Epilepsy
and had no addictions.
CLINICAL FEATURES: (O / E)
Lean patient with unremarkable general and systemic examination.
INVESTIGATIONS:
We investigated him by doing a T. M. T. which was positive
for ischemia and subsequently a coronary angiography. Coronary
angiography revealed severe Triple vessel coronary artery
disease with a well preserved Left ventricular function.
Routine blood investigations were done as a part of our
pre operative protocol which was within normal limits. (G6PD
enzyme assay was also done and activity was normal)
OPERATIVE PROCEDURE :
Patient was prepared and electively taken up for CABG after
5 days.
Total arterial revascularization of coronary arteries was
done.
Graft Details :
LIMA – LAD
RADIAL – OM
RIMA – PDA
Patient was shifted to SICU in a hemodynamically
stable condition. His ICU stay was uneventful and was transferred
to the ward on Post operativeDay 2 from where he was discharged
home on the 7th post op day.
The patient followed up with us after 3
months. He was asymptomatic. We carried out a modified Bruce
TMT on the patient [ as a part of regular post – operative
protocol] which was negative for Inducible ischemia.
This demonstrates successful arterial CABG
which is current trend in open heart surgery.
