Successful Triple Coronary artery bypass grafting surgery on a Beating Heart [At Nanavati Hospital]

A 69 yr old gentlemen, resident of Uttar Pradesh presented to us with complaints of severe chest pain radiating to Jaws and ( L ) upper extensity on walking approximately 100 meters.

The patient had been suffering from this disease for quiet some time but recently the attacks had become frequent and more severe.

He was a known hypertensive on regular medication and had smoked approximately
35 pack-years.

His clinical examination was unremarkable except sinus bradycardia [ HR – 56 ] which could be attributed to B – Blockers which he was consuming. Coronary angiography was done at Nanavati Hospital, which proved the diagnosis of coronary artery disease.[ triple vessel ].

Relevant blood investigations were carried out and patient was posted for CABG on March 27, 2004 after appropriate preparation.

He underwent off pump coronary artery bypass surgery by Dr. Pavan Kumar. Left internal mammary artery was grafted to LAD. OM2 & PLV received two separate reversed Saphenous vein grafts. The patient was shifted to SICU with stable hemodynamics& extubated on the same day.

Patient was shifted to the ward on second post – op day and discharged on sixth day.

Patient followed up with us after 3 months of OPCAB. Post op T.M.T. was negative for
Inducible Ischemia . This demonstrates successful Beating Heart [ MICAS ] surgery in elderly age group.
